Forms and applications for advisors

DAFgiving360™ offers two types of accounts:

  • Core accounts: The funds are invested in a selection of prescreened, competitive investment pools. There is no minimum contribution amount required, so it's simple to open an account and experience how easy it is to give through DAFgiving360.
     
  • Professionally managed accounts: For accounts with balances of $100,000 or more, advisors can create a customized investment portfolio and may charge an advisory fee to manage the assets in the account.*

Once your client is ready to open their donor-advised fund (DAF) account, you can assist them with the application process.

 



Effective February 25, 2026, DAFgiving360 has four updated forms for opening and managing a donor-advised fund (DAF) account:

  • New Account Application for a Donor-Advised Fund Account

  • New Account Application for Clients Associated with an Investment Advisor

  • Establish or Update Succession Plan

  • Update Donor-Advised Fund Account Information

Previous versions of these forms will not be accepted after February 24, 2026.

A donor opening a professionally managed account must recommend an independent investment advisor, who, if approved by DAFgiving360, will manage the assets contributed to the account. Advisors must meet certain eligibility requirements, including working with Schwab Advisor Services™, a business segment of The Charles Schwab Corporation, and agree to the Investment Advisory Agreement.
